[Probable serotonin syndrome due to linezolid and meperidine interaction].

El síndrome serotoninérgico es una reacción adversa farmacológica caracterizada por un súbito inicio de la tríada estado mental alterado, disfunción del sistema autonómico y alteraciones neuromusculares. Los síntomas pueden ir desde la agitación al coma, incluidas crisis mioclónicas, hiperreflexia, sudoración, taquicardia, hipotensión y fiebre. Puede suponer una complicación grave en el tratamiento con inhibidores selectivos de la recaptación de serotonina (ISRS), antidepresivos tricíclicos, inhibidores de la monoaminoxidasa (MAO) y otros fármacos potenciadores de la transmisión serotoninérgica. Es una reacción idiosincrásica y se ha descrito con el uso de monoterapia, combinación de fármacos, sobredosificación y con el uso de fármacos de abuso. Linezolid es un antibiótico del grupo de las oxazolidinonas que se comporta como un inhibidor débil, reversible y no selectivo de la MAO humana. Los inhibidores no selectivos o que inhiben el subtipo A de la enzima han sido asociados a casos severos de síndrome serotoninérgico, especialmente cuando se usan en combinación con opioides, ISRS o anfetaminas. A continuación se describe un probable caso de síndrome serotoninérgico por asociación de linezolid y meperidino en un paciente de cuidados intensivos.
